Final Approval for Cervid Importation: Import Permits and Certificates of Veterinary Inspection
Please use this checklist to ensure that you have all the required documentation before requesting an import permit. You must first have a completed and approved Utah Domesticated Cervidae Import Application and have completed all required testing. Packet of all applicable documentation

Proof of Ownership
Import Permits
All imports of cervids to Utah require an import permit from the Utah State Veterinarian’s office. An import permit will only be issued to an accredited veterinarian after all required information from the checklist above is provided to and examined by the Utah State Veterinarian’s office and the UDAF Elk Program. Cervid imports with Certificates of Veterinary Inspection (CVI) issued by electronic CVI (eCVI) platforms such as Global Vet Link or AgView are not exempt from the import permit requirement. The import permit number must be written on the CVI.
Certificate of Veterinary Inspection
All domestic cervidae must be accompanied by a Certificate of Veterinary Inspection (CVI) verifying that they have been inspected within 30 days prior to the date of shipment.
Veterinary Certification Statements
The accredited veterinarian must complete and sign the Veterinary Certification Statements for Import of Domestic Cervids to Utah document. This replaces the requirement for multiple statements on the CVI.
Test Results
All laboratory test results and tuberculosis test results must be provided to the Utah State Veterinarian’s office. For more information on the required tests, see the Utah Testing Import Requirements. An official ID for each imported animal must be listed on the test form.
Parasite Treatments
All animals must receive an anthelmintic (dewormer) approved for the treatment of Parelaphostrongylus tenuis (meningeal worm) within 60 days of import (unless the cervids are consigned directly to an approved slaughter establishment). The date of treatment and product name must be listed on the Veterinary Certification Statements form.
Identification
Each individual animal must be identified with at least two forms of identification. One form of identification must be an official radiofrequency ID (RFID) tag. Each individual animal must also be marked with a uniquely numbered visual form of identification such as a ranch or dangle tag.
Proof of Ownership
A paper which details each individual animal and their corresponding identification must accompany the shipment. This can be a Brand Inspection Certificate, notarized bill of sale, CVI or Bill of Consignment.
