How A Commercial Operator May Import
Freshwater Fish Into Utah

The following steps are necessary for a commercial fishery operator to receive permission from the Fish Health Program to import freshwater fish into Utah:

  1. Call the secretary of the Fish Health Program at (801) 538-7029 and request an Entry Permit to bring the desired freshwater fish into Utah.

  2. Provide your COR permit approval number and name, address and phone number to the office secretary. If not on the current FHP List, approval of your facility is required before shipment.

  3. Provide species name, quantity, size requested (inches & age), total poundage, name of the source where the fish or eggs will be procured. Fish may only be imported into Utah if the requested source is approved by the Department.

  4. The secretary will check the current Fish Health Approval List to ensure that the provider is permitted to ship freshwater fish into Utah.

  5. The purchaser provides an anticipated shipping date to the secretary. (Every effort will be made to complete entry permits during a one-day period; however, up to two weeks are authorized by law to verify approved status of the source).

  6. If the fish are classified as a warm water species the seller must complete an application for Warm Water Species Fish Health Approval. The Program Manager (prior to issuing an Entry Permit) must approve the application.

  7. The Fish Health Specialist contacts the seller and asks specific questions regarding the health of the fish to be imported. Information is passed to the Fish Health Specialists who evaluate the following:

    1. If fish are potential carriers of the Asian tapeworm. If so, then the importer/grower is informed of Triploid Verification (grass carp only) and Treatment or Testing Notice requirements (for all susceptible species);
    2. Completed copies of each that are sent to our office before final approval for shipment is made;
    3. If source waters currently have nuisance species or fish infected with the Asian tapeworm;
    4. Names of salmonids and other fish that occupy the water supply for the fish to be imported;
    5. Nature of rearing water since hatch (e.g., well, spring or open water etc.);
    6. The last inspection report.

  8. Just prior to issuance of the permit for warm water and non-salmonid fish, the Fish Health Statement is faxed to the importer, who has it completed and returned. A copy of the Fish Health Statement accompanies the warm water fish shipment. A Fish Health Statement is not required to accompany cold water fish shipments.

  9. The Fish Health Specialist and Program Manager review paperwork for discrepancies. The secretary prepares an Entry Permit. After successful review of the paperwork the Program Manager signs the Entry Permit.

  10. The Entry Permit is either faxed (with the statement for warm water fish) or mailed to the importer. The importer is informed on the fax coversheet that the permit and statement shall physically accompany the shipment. The importer submits this paperwork to the seller.

  11. The Entry Permit is good for 30 days. A copy of a Livestock and Fish Movement Report form accompanies the Entry Permit and associated paperwork.

  12. The purchaser calls the seller and makes arrangements for the shipment. The vehicle driver or airline is required to complete a Livestock and Fish Movement Report upon entry into Utah. The report is presented at any port of entry encountered upon entering Utah and/or at the airport.
